The application must be completed by the owner of the property himself and the assessment number, road and description
of the property must be mentioned correctly.
The reason for requesting a new number/new assessment should be clearly stated.
01 photocopies of the applicant's national identity card and an approved photocopy of the house plan belonging to the relevant proper
ty to get new numbers should be submitted and if there is no approved building plan, a letter should be submitted stating the same.
Rs.1000.00 per number and Rs.180 will be charged as value added tax.

Postage charges are to be paid on collection of new numbering letters by post.
A consent form should be submitted by the village officer and local records that there is no objection regarding getting a
new number stating that the owner is the government.
